Nurse Case Management

In addition to the free services offered by the clinic to our public safety officials including mental health counseling, physical therapy, health coaching, and exercise physiology, the Reserve Health location also offers nurse case management. A nurse case manager works alongside the provider team to facilitate healthcare plans and provide comprehensive care for patients. Our nurse case manager helps our public safety patients in several manners.

These include:

• Referrals to specialists required for clearance decisions or further diagnoses

• Establishing care with a primary care provider for all medical needs

• Coordinating preventative exams such as colonoscopies and mammograms

• Scheduling appointments for further evaluations such as imaging

• Staying in contact with the patient throughout treatment and facilitating any further assistance

Throughout the year, the nurse case manager has facilitated a wide range of appointments including referrals for surgeries, contacting specialists to expedite appointments when possible, coordinating sleep studies, scheduling appointments with primary care providers for medication prescriptions, and much more.
